Yes, I know you're crazy-busy (aren't we all?), but take 5 minutes and drop me a line. This isn't the kind of stuff that requires you to whip out a credit card or look up anything, unless you can't remember your own phone number, which these days is a sign of speed dial, not senility! So what're you waiting for? Oh yeah, the lowdown on the info I need.
Your name (first, maiden if applicable, last). Your mailing address, phone number, and email address. Spouse's name, if applicable, and the names and ages of your kids. If you have grandkids, tell us how many. Feel free to add a few lines telling us what you're up to these days, workwise or leisure-wise.
